Agilix Maternal Nutrition and Safety

One Health is excited to offer the Agilix Maternal Nutrition and Safety Program, which focuses on the role of whole-person care in both maternal and fetal health.  The program emphasizes continuous monitoring and care oversight throughout the prenatal and postnatal periods as Agilix’s Registered Dieticians provide personalized nutritional support for expectant mothers by combining remote patient monitoring with virtual care. The program alerts care or case teams to issues that require attention, ultimately improving care outcomes, enhancing member experience and safety, reducing avoidable utilization and addressing modifiable drivers of disparities in care outcomes.

Core Program Elements

• Comprehensive ‘Personalized’ Nutritional & Safety Education: Registered Dietitian led virtual consultations covering nutrition & Social Determinants of Health needs assessment, personalized nutrition advice and goal setting; health literacy and maternal safety education

• Longitudinal Patient Monitoring & Risk Assessment: Connected weight scale to monitor appropriate weight gain and manage maternal nutrition

• Risk Alerting & Escalation System: Coordinated Clinical or SDOH/Case management escalation based upon ongoing patient assessment to address any existing or emerging issues in a timely manner

FAQs

How does the program work?

After Agilix receives a referral from your provider, a representative from Agilix will contact you to get you enrolled in the program and set up a phone appointment for you to talk to a dietitian. Your dietitian will call you when it’s time for your appointment and talk about your diet care plan and work with you to create a personalized plan for a healthy pregnancy.

Will this program cost me anything?

Depending upon your insurance, there might be cost sharing/a co-pay. Contact your insurance provider to verify remote physiological monitoring is a covered benefit in your plan. 

Do you only do therapy?

At MindHealthy, we provide support for care management services, administration of validated rating scales, and development of individualized treatment plans. We collaborate with psychiatric nurse practitioners for consultation and provide brief interventions, utilizing evidence-based techniques. While we may offer medication recommendations upon requests, we do not engage in medication prescribing.

When can patients be referred to the program?

Patients can be referred to the program as soon as the pregnancy confirmation appointment. 

Will the Agilix dietitian talk to my doctor and/or care team? 

Yes, the Agilix dietitian will work closely with your doctor and your care team, so they’re all up to date on your health and progress.

How often should I use the weight scale? 

You should use the scale every day to help us keep an eye on your health, you will speak with the dietitian twice a month so we can support your pregnancy and monitor you closely.
