Independent Providers
We understand that the burden of transformation, value, and operations for an independent provider have increased substantially over the last decade; yet independent providers are critical for patient access and quality. One Health partners with independent providers to assess the practice and offer options that will best support the provider/practice and both short and long-term goals.
ACOs/CINs
One Health partners with existing ACOs and CINs to maximize operations and deliver on value. Through physician leadership, operational expertise, and a transformative culture ACOs and CINs benefit with enhanced contracting, support services, and a tailored approach to driving success in value.
Hospital/Health Systems
One Health partners with hospitals and health systems to align providers and aggregate populations through aligned vision and intentional culture. We work with practices to identify physician leadership, transform culture, and serve as a liaison between hospital administration and their physician group(s).

We believe that the current state of healthcare is unsustainable, and that payers and providers must partner together to deliver new models of care and financial models that support providers and payers alike.

One Health partners with small businesses to create access through on-site care, executive health programs, and access to our One Health Network of Family Medicine, Internal Medicine, Pediatrics, and OB-GYN Providers. Partners
One Health is proud to partner with the following organizations to deliver Modern Primary Care and to provide stability through these transformative times in healthcare.
One Health has engaged in a unique model with Advocate that allows One Health providers to practice independently, while leveraging important services through the health system. This partnership allows physician autonomy while benefiting from the scale of a health system.
One Health is proud to partner with C3 HealthcareRx to deliver additional support services to One Health providers and patients. Current services include the collaborative care model through which One Health providers can reach out and seek behavioral health support for their patients. Patients are generally contacted the same- day/next-day and able to get support needed. Additional services will be added in the future.
One Health partners with CHESS as a provider of value-based services, including care navigation, contracting, and data/analytics.
