Clinical Pharmacy Services

One Health is proud to offer clinical pharmacy services for patients throughout the One Health system. The pharmacy mission is to improve patient health outcomes through optimal medication selection and use, prevention of adverse drug events and improved medication access and adherence. Pharmacists and pharmacy technicians achieve this mission through direct patient care, population health management and education of patients, providers and staff.

We recognize that each patient is unique. Our goal is to provide each patient with the medications that work best for their health, their lifestyle and their body.

Clinical pharmacist practitioners

Direct Patient Care

Clinical pharmacist practitioners (CPPs) meet with patients to manage chronic medical conditions such as diabetes, hypertension, hyperlipidemia, osteoporosis, COPD, and many others. CPPs also help patients navigate transitions of care from the hospital or skilled nursing facilities to home. This can be a time with many medication adjustments which are important for keeping patients stable at home. In addition to medication management, pharmacists can assist with medication administration counseling and continuous glucose monitors. Pharmacy technicians work with prescribers, patients, and insurance companies to ensure patients have access to their medications at an affordable price.

Population Health Management

The pharmacy team also works to identify patients who are not meeting their health care goals or who are unable to take their medications as prescribed. This may happen for a variety of reasons including side effects, unclear instructions, or cost. We aim to address the needs of these patients individually but also to implement system-based strategies to better support all our patients.

We provide education for patients on their individual medications but also ensure providers and staff have the most up-to-date information on medications so they can provide the best patient care.

Megan Coleman

PharmD, BCPS, CPP

Megan is a graduate of Wingate University School of Pharmacy.  She completed pharmacy residency training at the Ralph H. Johnson VA Medical Center in Charleston, SC in 2012.  She then joined the faculty at WUSOP and has maintained a clinical pharmacy practice at the Matthews Free Medical Clinic and Novant Health Family Medicine – Arboretum.  Megan joined One Health full-time in February 2024 and provides chronic disease state management focused on diabetes, HTN, cardiovascular risk reduction and women’s health in the Charlotte market.

Jenna Picton Theroux

Jenna is a graduate of Wingate University School of Pharmacy. She completed two years of residency training at New Hanover Regional Medical Center and Lahey Hospital Medical Center.   She has 5 years of clinical experience practicing in ambulatory care and HIV, most recently at Eastowne Family Physicians. She provides chronic disease state management, primarily diabetes, HTN, obesity, cardiometabolic risk at the NoDa location.

Geoff Mospan

Geoff graduated from Ohio Northern University and completed residency training at the University of Toledo Medical Center. Geoff joined Wingate University in 2014 and maintained an inpatient practice for several years. Geoff will be at Prosperity office and will provide transitional care services for patients recently discharged from the hospital.  He has a special interest in cardiology, infectious disease, pain management and has experience with substance use disorders.

Kira Harris

Kira Harris serves One Health as the Director of Clinical Pharmacy Services. In addition to providing patient care, she is responsible for implementing clinical pharmacy services and value-based projects throughout One Health.
Kira is a graduate of the University of North Carolina in Chapel Hill School of Pharmacy. Following residency training specializing in ambulatory care, she pursued a career in academia at Wingate University School of Pharmacy. She transitioned from pharmacy education to family medicine residency education in 2019. She has experience in lecture- and practice-based teaching for pharmacy and medical students and residents. She has maintained a clinical practice as a clinical pharmacist practitioner for over 15 years. Her clinical interests include diabetes, cardiovascular risk reduction and osteoporosis management.

FAQs

What is a Clinical Pharmacist Practitioner?

North Carolina is one a handful of states that allows for the clinical pharmacist practitioner (CPP) license.  Very similar to a nurse practitioner or physician assistant, CPPs work under the supervision of a physician(s) and within a specific scope of practice and are given prescriptive authority.   This means they can manage conditions diagnosed by a physician, prescribe medications, order labs and provide follow-up.  To qualify for a CPP license, pharmacists must have either residency training or similar clinical experience and maintain a certain amount of continuing education.

Why would I see a CPP?

CPPs work in conjunction with your provider to help manage your medical conditions. Pharmacists are experts in medication options. They are able to focus on selecting medications that are best for you.  There are many reasons you may be referred to a CPP. If you are not reaching your health goals (weight loss, high blood sugar or blood pressure), you may benefit from meeting with a pharmacist. If you are having side effects from your medications or are concerned about medication interactions, you may benefit from pharmacy services.
