David Cook

Dr. David Cook has leveraged his 35 years in medicine to create sustainable, scalable, and effective primary care platforms and integrated delivery models with a laser focus on quality, cost, provider wellness, and consumerism. He is a practicing physician, entrepreneur, innovator, and healthcare futurist. His core strength is seeing what is needed, building a plan, and inspiring his teams and others to execute the vision. Dr. Cook is considered a thought leader in reimaging and reengineering primary care, value-based care, and population health.

His accomplishments include founding one of the largest and most progressive primary care practices in Charlotte, NC, co-creating the One Health partnership with Advocate Health, creating a family medicine residency program, serving as the medical group president for the fourth largest medical group in the country, serving as the health system leader in population health, value-based care, innovation, and strategic growth, and serving as chief of staff. He founded a safety net clinic twenty-six years ago to serve the most vulnerable individuals. Dr. Cook still serves as the medical director for this clinic.

Dr. Cook works with medical groups, independent practices, specialists, primary care, hospitals, healthcare systems, government, payers, consultants, technology, venture capital, and private equity. Through strong leadership and vision, he has been instrumental in creating systems, processes, and platforms that value quality and service while reducing the total cost of care. His work and passion center around supporting providers and health systems to improve the health and well-being of patients and communities. He couples this passion with an ability to help payers, the government, and businesses solve the ever-increasing complexity and cost of obtaining healthcare for those they employ and insure.

Dr. Cook’s wife and three adult children are the joy of his life. He also enjoys fly fishing, boating, golf, hiking, travel, motorcycles, and music. He is an avid student of health equity, American healthcare, global healthcare delivery, nutrition, exercise, and longevity. He is the co-founder and co-CEO of Modern Primary Care Consultants and One Health.

Ehab Sharawy

Dr. Ehab Sharawy, co-founder and C-CEO of Modern Primary Care Consultants, is a board-certified OB/GYN with decades of experience building physician practices. His patient-centered approach rests on a foundation of access, quality, safety, and satisfaction for both the patient and provider. Dr. Sharawy believes the strength and success is in the people. He believes in inspiring them to be their best and that will ultimately achieve the best outcomes. He is thrilled to share his experiences with physicians and healthcare stakeholders to create a sustainable healthcare delivery model.

Dr. Sharawy spent 16 years as a senior physician leader in a large integrated healthcare system/ He held numerous roles that included leading strategy and operations for women’s services at the sub-market, market, and system levels. Dr. Sharawy is a national thought leader in Women’s health focused on the disparities in healthcare for women and the solutions to reduce these disparities. He has committed his whole career on creating a sustainable healthcare environment by restoring the joy of medicine for providers, working collaboratively with all the stakeholders in healthcare, and aggregating populations to ultimately take care of the whole of the community and reduce the total cost of care. Dr. Sharawy successfully combines the day to day operations with the vision and strategy to make transformative change. By using Modern Primary Care as the center and core of these principles, he believes that we can set the foundation for sustainable delivery of optimal health for the United States. He continues to be a practicing OB/GYN.

Dr. Sharawy graduated with highest honors from the Georgia Institute of Technology. After attending the Medical College of Georgia on a full-merit scholarship, he completed his residency at the University of Florida Health Shands Hospital in obstetrics and gynecology. He was voted Resident of the Year by the medical school class of 1997 and received numerous teaching awards.

He is the co-founder and Co-CEO of Modern Primary Care Consultants and One Health.

He and his wife Patricia have two children, Max and Carson. Max is in the first year of medical school, and Carson is completing her MBA.

Mark Collins

Dr. Mark Collins understands the complex challenges facing today’s healthcare providers, having served over two decades in a physician leadership role. A board-certified Family Physician, he has practiced in Charlotte, NC as a full-time primary care doctor and in multiple leadership positions. In these roles, Dr. Collins had responsibility for integrating numerous primary care and specialty practices across a large healthcare system.

Over the past several years, Dr. Collins has successfully led hundreds of primary care providers through strategic growth initiatives, compensation plan changes, care team design, and provider wellness. Through trusting professional relationships and transparent communication at each of the 60+ clinic locations, he developed a culture of high quality, consumerism, and responsible autonomy.

Dr. Collins is highly motivated to prevent provider and team member burn out. As a physician still engaged in direct patient care, he understands the calling that providers have for their patients and recognizes the sacrifice the career demands that can lead to a personal cost to providers and their families. He is challenged to restore the joy of medicine for those he serves. Modern Primary Care Consultants will serve as a catalyst for changing the delivery of health and wellness throughout the nation.

Dr. Collins knows the current paradigm must change for health delivery to be sustainable, affordable, and FOR ALL. The Modern Primary Care Platform, with its myriads of clinical resources and consumer centric approach will help deliver on that promise. As CSO, he is committed to partnering with innovative companies to further enhance the Modern Primary Care Platform, knowing that patient centered integration and collaboration only makes the delivery of health stronger. By keeping the provider-patient relationship at the heart of the model, he works daily to create the best environment to deliver on the Modern Primary Care Consultants Mission.

In addition to his commitment to his professional endeavors, Dr. Collins enjoys spending time with his family. He has been married to his wife, Shannon, for twenty-nine years. Together they share seven children and one grandson. Dr. Collins is passionate about his faith, family, and adoption.

Dr. Jim Schaffer has dedicated over 30 years to his roles as a physician, US Naval Officer, healthcare executive and leader. Throughout his career, he has demonstrated an unwavering commitment to delivering exceptional patient care while cultivating a collaborative, supportive environment for his colleagues. Known for his attention to detail and problem-solving skills, Dr. Schaffer excels at translating complex issues for stakeholders and leveraging technology to improve operational efficiency.

He spent 12 years in the US Navy as a Family Physician and Flight Surgeon. His time in the Navy was pivotal in shaping his leadership style, instilling in him the values of servant leadership, teamwork, and the critical importance of trust in building effective teams. He honed his ability to address clinical and operational challenges with precision, translating vision into actionable steps that drive success.

In 2006, Dr. Schaffer co-founded a family medicine practice in Charlotte, growing it into one of the largest practices in the area. As he transitioned into physician leadership roles, including as a market lead at his previous organization, he continued to embrace the diverse and dynamic nature of family medicine while applying his keen problem-solving abilities to organizational challenges and advance healthcare delivery.

Since joining One Health in 2021, Dr. Schaffer has held several key leadership roles including Regional Medical Director, Chief Medical Officer, and now Chief Operating Officer of One Health and Modern Primary Care Consultants. In his current role, he oversees clinic operations, strategic growth, provider recruitment, communications, and personnel management. His leadership is defined by a commitment to empowering healthcare providers and teammates, fostering environments that encourage both personal and professional growth.

Dr. Schaffer’s diligence, expertise in resolving complex issues, and collaborative approach have been instrumental in shaping an organization that equips providers with the tools and support necessary for success. His journey, from the Navy to healthcare executive, reflects a lifelong dedication to service, teamwork, and a detailed approach to solving the complex challenges of healthcare.

He is married to Suzanne, a commercial real estate attorney, and together they have two daughters who are their greatest source of joy. In his free time, he enjoys traveling, snow skiing, and spending time at the beach.

Scott Vogler

Dr. Scott Vogler brings over 25 years of dedicated service as a family physician, with a career that spans patient care, healthcare education, and the development of diverse, high-performing teams to deliver care to underserved communities. His unwavering commitment to his patients, colleagues, and the broader healthcare system is reflected in his many achievements and continues to shape his impact on healthcare. As a medical student at Wake Forest University (WFU), Dr. Vogler’s dedication to patient care and teamwork earned him the distinction of being elected by peers and faculty to receive a prestigious full-tuition scholarship. He continued his leadership as a co-chief resident during his family medicine residency at WFU. In 2001, Dr. Vogler joined a rural/suburban practice in his hometown, initially consisting of four physicians. Alongside full-time practice, he established a rural residency rotation for WFU Family Medicine, mentoring 10 residents annually, and further expanding his impact by teaching medical and physician assistant students.

Over the next two decades, driven by a mission to enhance patient care and provider development, Dr. Vogler grew the practice to 28 providers across three locations. Under his leadership, the practice pioneered innovative care delivery models and became the region’s largest primary care office, all while maintaining fiscal responsibility and fostering a nurturing environment for educating future healthcare providers. Concurrently, he led the Triad Health and Wellness Clinic (501c3), ensuring access to healthcare for all patients, regardless of insurance status or income level. Dr. Vogler assumed leadership roles within his practice and then as a regional and market leader in primary care for a large health system.

In 2021, Dr. Vogler joined One Health as a Regional Medical Director and then the Chief Medical Officer. In this role, he continues to focus on delivering high-quality care and fostering provider development. Over the past three years, he has overseen the expansion of 10 new practice sites across urban, suburban, and rural areas and facilitated the integration of more than 40 healthcare providers, spanning family medicine, pediatrics, and rheumatology. These efforts, involving complex collaborations across multiple health systems and electronic medical record transitions, reflect his ongoing commitment to advancing healthcare access and improving patient outcomes in diverse communities.

Dr. Vogler remains deeply committed to supporting the entire healthcare team, emphasizing provider wellness, and never forgetting that it is people who make us all successful.

Above all his professional achievements, Dr. Vogler finds his greatest joy in loving and supporting his wife, two daughters, and family. He recharges by staying active outdoors, enjoying golf and basketball, and taking time for quiet reflection on a life he considers truly blessed.

Kelly Garrison

Kelly Garrison brings more than 15 years’ experience in healthcare, program design and implementation, entrepreneurship, value-based care, population health, and change management to support providers, systems, payers, businesses, government entities, and communities who are striving to succeed and thrive during this evolving time of transformation within the healthcare system.

Kelly successfully led the transition of a regional not for profit organization that served the Medicaid population for more than 20 years through the formation of a joint venture, Emtiro Health. As one of the Nation’s only Medicaid focused population health entities, Emtiro became one of the largest and highest performing care management entities in the Managed Medicaid space. Through this experience and a deep commitment to vulnerable populations Kelly is seen as a national thought leader in population health, especially for underserved populations.

Through years of leading organizations and supporting some of the nation’s most complex and vulnerable patients, Kelly understands the need to bring together multiple sectors of the community and healthcare ecosystem in a collaborative manner to support patients. Additionally, Kelly has a passion for supporting providers and their wellbeing as the most critical point of our healthcare delivery system. She brings systems and supports together that facilitate the patient-provider relationship while fostering provider wellbeing. Kelly’s focus remains on right-sizing population health activities to the needs of the population and community, leveraging technology, integrating services, and reducing infrastructure costs to reinvest in primary care.

Kelly earned a BS in Sports Medicine from Lynchburg College and a Dual master’s degree in business administration and healthcare administration from Pfeiffer University. She serves on several community boards focused on youth, equity, and health; and enjoys serving as a leadership mentor to women seeking professional advancement in healthcare and other industries.

Kelly and her husband John, have two boys, Logan who golfs competitively; and Davis who swims and plays tennis. In her free time, Kelly enjoys baking, spending time at Hilton Head, walking, and reading.

Mark Lautzenheiser

Mark Lautzenheiser has almost 25 years experience in the health care industry and has served in executive positions in a variety of organizations. His work has been with commercial payers, hospitals, healthcare systems, physician groups and specialty staffing with expertise in value-based care, financial modeling, budgets, strategy, innovation and data analytics.

Mark previously served as CFO CHESS Health solutions where he joined during the start-up phase of the organization and helped it grow to nationally recognized value-based care provider. During that time CHESS achieved tremendous success in its contract with Atrium Health Wake Forest Baptist as its flagship organization.

Throughout his career he has built finance and accounting teams from the ground up and has implemented various accounting and data analytics systems. A specific area of expertise is creating advance financial models to evaluate contract performance and build long-term financial projections. Mark has the unique ability to distill complex financial information into an easy-to-understand format.

Mark brings to Modern Primary Consultants a strong ability to maximize revenue while managing cost that helps to achieve a strong return on investments. His deep knowledge of total cost of care management programs and risk management strategies, along with his ability to collaborate with clinical operations, helps achieve success in value-based contracts.

Mark and his wife Danielle have 1 daughter Zella who is in high school. In his spare time, he enjoys weekend trips to the mountains or beach with his family.

Dr. Kira Harris is a dedicated Clinical Pharmacist Practitioner with over a decade of experience in primary care and academia. She is passionate about optimizing medication use and access, ensuring that all patients receive the best possible care. Dr. Harris holds a Doctor of Pharmacy degree from the University of North Carolina Eshelman School of Pharmacy in Chapel Hill and has completed two years of specialized residency training in primary care.

Following her training, Kira served on the faculty for 10 years at Wingate University School of Pharmacy, where she was instrumental in shaping the next generation of pharmacists. Her academic contributions included mentoring students, conducting research, and developing innovative curricula focused on enhancing patient care through effective medication management. During this time, she built a clinical practice in family medicine focused on cardiovascular risk reduction, managing diabetes, hypertension, hyperlipidemia and obesity.

Dr. Harris’s core values center on equitable healthcare access, patient education, and the importance of collaboration among healthcare providers. She is committed to implementing innovative pharmacy strategies that improve patient outcomes, fostering a holistic approach to healthcare that empowers individuals and communities alike.

In her current role, Dr. Harris continues to champion the integration of pharmacy services in primary care settings, advocating for practices that enhance medication safety and efficacy. She oversees a team of clinical pharmacists and pharmacy technicians dedicated to optimizing medication use, reducing barriers to evidence-based treatments and enhancing the patient experience. Her extensive expertise and unwavering commitment to patient-centered care make her a vital asset in the field of pharmacy and healthcare.

As a leader of multiple primary care practices in a large integrated health system, Shannon assisted with the development and implementation of key performance solutions aimed at modernizing healthcare delivery while improving access to high-quality care and increased provider satisfaction. She was instrumental in integrating pharmacy and mental health services into primary care practices, significantly broadening the scope of care provided to the communities they serve.
Fostering a culture of forward-thinking healthcare delivery, Shannon began offering direct patient care services by partnering with local businesses to provide enhanced and cost-efficient access to care. With her deep understanding of the healthcare landscape, Shannon has served on numerous leadership committees where she has been instrumental in shaping organizational policies and strategies aimed at consistently delivering improved patient outcomes while increasing operational efficiency.
As an advocate for ethical healthcare leadership, she has been an educator, teaching medical ethics and terminology at the college level, inspiring the next generation of healthcare professionals to prioritize ethical decisions-making skills that are essential in today’s complex healthcare environment. Beyond her professional achievements, Shannon is an active volunteer with numerous organizations that raises awareness and funds to support cancer research, advocacy and community health initiatives.
Outside of her professional life, Shannon enjoys time with her husband, Robbie, and their 5 children and 7 grandchildren.

Jessica Reisner is a dedicated Certified Physician Assistant at One Health Family Medicine Rea Village and serves as One Health’s Collaborative Care Model Lead Clinician and Co-Director of the Advance Practice Provider (APP) Advanced Training Program. With over 14 years of healthcare experience across a variety of clinical roles, she brings an intentional focus on patient experience, clinical excellence, collaboration, provider wellness, and servant leadership to every aspect of her work.

A native of the Charlotte area, Jessica feels privileged to provide compassionate, high-quality care to her community. She has a tremendous amount of respect for Family Medicine and the unique opportunity it affords her to comprehensively treat the whole patient, considering their physical, mental and emotional well-being. Jessica believes in empowering patients to actively participate in their healthcare, with the ultimate goal that each one leaves with a plan, a smile, and the feeling of being truly heard and understood.

Jessica’s journey with the Behavioral Health Collaborative Care Model began over a decade ago when she wrote her thesis on the subject, delving into its potential to revolutionize patient care through integrated, collaborative approaches. Over the years, she has continued to advocate for, and most recently contribute to, the refinement of this model, ensuring that it meets the needs of patients and healthcare providers alike.

Throughout her career, Jessica has gained a deep understanding of the evolving healthcare landscape and vital role APP’s play in delivering exceptional, patient-centered care. As Co-Director of the APP Advanced Training Program, her mission is to cultivate and empower new graduate APP’s and promote their professional growth, clinical excellence, and commitment to lifelong learning and service. Above all, she aims to build confidence in every new provider entering the field.

Beyond her professional career, Jessica enjoys quiet moments with a warm cup of coffee, spontaneous kitchen dance parties, traveling, live music, and most importantly, spending time with her family and friends. She and her husband, John, are proud parents of three amazing children and embrace life’s beautiful chaos.

Alisa Folds

Alisa Folds is a seasoned professional with extensive experience in leadership, business, and healthcare. As a Recruitment Specialist at One Health, she excels in collaborating with cross-functional teams to enhance business operations, manage projects, and implement technology solutions tailored to organizational needs. Alisa is responsible for sourcing, interviewing, and hiring top talent to ensure the continuous growth of the organization. Her strong analytical and problem-solving skills, coupled with a passion for leveraging technology to streamline processes, have made her an invaluable asset to the organization.

In her current role, Alisa works on recruiting providers for One Health to ensure continuous growth. She also implemented a One Health teammate onboarding program with Sarah Boles that runs monthly. Additionally, she manages internal and external communications as well as website management.

Alisa holds an MBA and a B.S. in Business Administration Information Technology Management from Western Governors University, as well as an Associates of Science in Radiography from Northern Virginia Community College. Currently, she is pursuing a Graduate Level Certification in Foundations of Economic Analysis at The University of North Carolina at Greensboro.

Outside of her professional life, Alisa enjoys spending time with her two dogs and her husband outdoors, spending time with her family, and reading.
