The One Health Mission

There’s no denying that healthcare workers are the backbone of healthcare. You are dedicated individuals who work tirelessly to care for patients and ensure that they receive the best possible care. The COVID-19 pandemic highlighted the critical role that each of you play in our communities, and the importance of supporting them. We thank each of you for what you do each and every day!

At the heart of the healthcare system is PRIMARY CARE. Primary Care is where patients go for routine check-ups, prenatal and obstetrical care, preventive care, and treatment for minor illnesses and injuries. Primary Care is also where patients receive support for managing chronic conditions and navigating complex medical, social, and emotional issues. And yet, primary care is often under-resourced and undervalued.

That’s where our value-based primary care platform comes in. At One Health, we believe that primary care is the foundation of a healthy community, and we’re committed to enhancing the patient experience, improving the health of individuals and populations, and reducing medical costs. We also recognize that we can’t do this without a laser focus on team member wellness and care for the entire patient population in our communities.

At One Health, we support our primary care team deliver high-quality, patient-centered care. With our partners, Advocate (Atrium) Health and CHESS, we use data analytics to identify patients at risk for poor health outcomes. Most importantly, we give our providers the tools and resources they need to deliver targeted interventions, better care for our patients, and a team-based approach to lessen workload at an individual level.

We prioritize patient engagement and empowerment, recognizing that patients who are actively involved in their care are more likely to achieve better health outcomes.

We also recognize that our teammates are human too. That’s why we prioritize teammate wellness. We encourage our team to prioritize health and wellness. By managing stress, we can avoid burnout. We believe that by taking care of our team, we can improve the quality of care they deliver and ensure that they’re able to continue providing care for years to come.

Ultimately, the One Health goal is to create a healthcare system that works for everyone. We believe that a robust Primary Care platform is the key to achieving this goal and lowering the total healthcare spend (total cost of care). We are committed to ensuring that Primary Care providers have the support they need to deliver high-quality care to all patients, regardless of their background or circumstances. By promoting Primary Care and working together, we can create a healthier more equitable future for all.
